Personal Care Services¶
Personal Care (PC) services are available to assist an eligible member to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs) in the member's home, place of employment or community.
Eligibility¶
- Care recipient must have a qualifying disability
- Relationship: parent, child
Questions to ask the program¶
These are questions to ask the program, not a verdict on your eligibility. Confirm the current rules directly with the program.
- Does the person receiving care live in West Virginia? — Residency is usually the first thing a program checks.
- Does your relationship to the person — parent or child — count for this program?
- What counts as a qualifying disability or level of care need, and what documentation shows it?
- Is there a caregiver-specific department, coordinator, or support line? Ask for it by name. — Many agencies have one that isn’t advertised on the website.
- What documents do you need from us to start an application?
- Is there a waitlist right now, and how long is it?

How to apply¶
- Fill out and sign a Personal Care-Medical Necessity Evaluation Request (PC-MNER) form and provide it to your doctor (MD or DO), physician's assistant, or registered nurse to complete, sign, and submit to Acentra Health-PC, 1007 Bullitt St., 2nd Floor, Charleston, WV 25301 (toll-free 1-866-385-8920, fax 1-844-794-6729). An Acentra Health registered nurse will then contact you to schedule a medical eligibility assessment in your home.
