How to apply¶
- Enroll in Medicaid (STAR+PLUS, STAR Health, STAR Kids, or Traditional/FFS Medicaid) and ask your MCO, DSHS, HHSC caseworker, or LIDDA about CFC; to switch to the Consumer Directed Services option, complete Forms 1582, 1583, 1584, and 1586 with your caseworker.
