Connecticut Adult Family Living (Structured Family Caregiving)

Connecticut's Adult Family Living program pays family caregivers a daily stipend through the Connecticut Home Care Program for Elders (CHCPE) Medicaid waiver to provide in-home care to elderly individuals aged 65+ as an alternative to nursing facility placement. The caregiver must live with the care recipient. Spouses may NOT serve as caregivers. A provider agency coordinates services, provides caregiver training, and conducts regular oversight visits.

Pays family caregivers a tax-free daily stipend for providing in-home care to Medicaid-eligible elderly individuals aged 65+ who require a nursing facility level of care. The caregiver receives 50-65% of the Medicaid daily rate. CHCPE has both Medicaid-funded and state-funded categories: Medicaid 1915(i) income limit 150% FPL ($1,956.25/mo in 2025), 1915(c) waiver income limit 300% FBR ($2,901/mo in 2025). Medicaid asset limit $1,600 single/$3,200 couple. State-funded has no income limit but asset limit $47,376 single/$63,168 couple (2025). Community Spouse Resource Allowance up to $157,920. Wait lists may form when enrollment slots or funding are limited. CT's Community First Choice Program also offers self-directed personal care. Spouses may NOT serve as caregivers.

Eligibility

  • Household income at or below 150% of the Federal Poverty Level
  • Asset limit of $1,600
  • Care recipient must be at least 65 years old
  • U.S. citizenship or qualified immigrant status required
  • Care recipient must have a qualifying disability or need for assistance
  • Caregiver must live with the care recipient
  • Must be a resident of CT
  • Must be Medicaid-eligible
  • Must require a nursing facility level of care
