How to apply¶
- Use the state Medicaid application information page if you need Medicaid or have an immediate need for personal care services.
- For a person age 18 or older, the New York Independent Assessor Program completes a community health assessment and clinical appointment to determine PCS eligibility.
- For a person under age 18, the doctor sends a completed Physician's Order for Services to the local social services district or managed care plan. The district or plan completes the community health assessment.

Care-needs rules and exceptions¶
- The minimum-needs rules took effect September 1, 2025. For PCS and CDPAS they apply to Medicaid recipients age 21 or older; for enrollment in a Medicaid Advantage Plan or Managed Long Term Care Plan they apply at age 18 or older. - One route requires at least limited assistance with physical maneuvering in more than two activities of daily living. [New York Department of Health]
- For a person with a dementia or Alzheimer's diagnosis, the alternative requires at least supervision with more than one activity of daily living. [New York Department of Health]
