New York Consumer Directed Personal Assistance Program (CDPAP)

The Consumer Directed Personal Assistance Program (CDPAP) is a New York State Medicaid program that allows eligible individuals who need home care services to recruit, hire, train, supervise, and dismiss their own personal assistants — including family members and friends. The consumer (or their designated representative) directs their own care.

Allows Medicaid-eligible individuals to hire, train, and direct their own personal assistants (PAs), including family members (except spouses) and friends. PAs provide personal care, home health aide services, and/or skilled nursing tasks as authorized. There is no maximum number of hours -- consumers may receive up to 24 hours per day, 7 days per week, based on assessed need. 2026 CDPAP rates effective 1/1/2026: NYC Basic $27.34/hr, Hard to Serve $29.34/hr, Live-In $354.63/day; Nassau/Suffolk/Westchester Basic $26.46/hr, HTS $26.97/hr, Live-In $338.10/day; Rest of State Basic $24.73/hr, HTS $27.07/hr, Live-In $320.94/day. Public Partnerships LLC (PPL) is the sole statewide fiscal intermediary (effective April 1, 2025) handling payroll and benefits for all PAs.
