Montana Community First Choice (CFC) / Big Sky Waiver¶
Montana implemented the 1915(k) Community First Choice state plan option with self-direction, providing personal assistance services as a Medicaid entitlement. The Big Sky Waiver and Personal Assistance Services program also allow consumer-directed care. CFC is an entitlement (no waitlist), though the Big Sky Waiver may have waitlists for additional services.

- Care recipient must have a qualifying disability or need for assistance
- Must be a resident of MT
- Must be Medicaid-eligible
- Must require a nursing facility level of care
