Medicare GUIDE (Guiding an Improved Dementia Experience) Model¶
The GUIDE Model is a voluntary national model designed to support people living with dementia and their unpaid caregivers. It aims to improve quality of life, help people remain in their homes longer, and provide respite and education for caregivers.
Key benefits include: 1) A respite service stipend of up to $2,500 per year for in-home, adult day, or facility-based care. 2) 24/7 access to a support line. 3) Comprehensive care coordination and management. 4) Caregiver education and skills training. 5) Screening for health-related social needs and community referrals.
