Maryland Community First Choice (CFC) / Community Options Waiver¶
Maryland implemented the 1915(k) Community First Choice state plan option, providing self-directed personal assistance services as a Medicaid entitlement. The state also offers the Community Options Waiver and Community Pathways Waiver with consumer-directed options. CFC is an entitlement with no waitlist for CFC specifically, though the HCBS waivers may have waitlists for additional services.

Eligibility¶
- Care recipient must have a qualifying disability or need for assistance
- Must be a resident of MD
- Must be Medicaid-eligible
- Must require a nursing facility level of care
