1915(i) State plan Home and Community-Based Services provides home and community-based services for eligible individuals with severe mental illness and/or substance use disorder.

These are questions to ask the program, not a verdict on your eligibility. Confirm the current rules directly with the program.
- Does the person receiving care live in Kentucky? — Residency is usually the first thing a program checks.
- What counts as a qualifying disability or level of care need, and what documentation shows it?
- Is there a caregiver-specific department, coordinator, or support line? Ask for it by name. — Many agencies have one that isn’t advertised on the website.
- What documents do you need from us to start an application?
- Is there a waitlist right now, and how long is it?

- How do I apply for 1915(i) RISE? You can: Ask your provider or case manager about 1915(i) RISE. Call: 502-564-9189. Email: 1915iRISEInitiative@ky.gov. A 1915(i) RISE Assessor will contact you after a referral is submitted, complete a pre-screening with you to understand your needs, and schedule a formal Assessment.
