The Program of All-Inclusive Care for the Elderly (PACE®) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.

These are questions to ask the program, not a verdict on your eligibility. Confirm the current rules directly with the program.

  • Does the person receiving care live in Indiana? — Residency is usually the first thing a program checks.
  • Is the person receiving care in the covered age range (55 or older)?
  • Is there a caregiver-specific department, coordinator, or support line? Ask for it by name. — Many agencies have one that isn’t advertised on the website.
  • What documents do you need from us to start an application?
  • Is there a waitlist right now, and how long is it?

How to apply

  • There is no single statewide enrollment portal; PACE is delivered by local PACE organizations, each serving specific counties or zip codes. To enroll, contact the PACE organization serving your county directly (see the Indiana PACE Programs directory of addresses and phone numbers by service area on the DDARS PACE page). Applicants must be certified to need nursing facility level of care and able to live safely in the community at the time of enrollment.
