Eligibility¶
- Household income at or below 150% of the Federal Poverty Level
- Care recipient must have a qualifying disability
- Care recipient age: under 21

- Does the person receiving care live in Illinois? — Residency is usually the first thing a program checks.
- What are the current income and asset limits, and whose income counts? — Our record lists 150% of the federal poverty level — limits change, so confirm the current figures.

- What counts as a qualifying disability or level of care need, and what documentation shows it?

How to apply¶
- Referrals may come from hospitals, physicians, schools, nursing agencies, and the OA website. An assessor at the OA performs the person-centered comprehensive assessment that is sent to the MA, along with supporting documentation, including medical reports. The OA submits the completed eligibility enrollment packet to the MA for review.
