How to apply¶
- The Elderly waiver application is part of the full Medicaid application: check 'Services to remain in your home' and Elderly waiver in Appendix A. Apply online through the Iowa HHS Benefits Portal, by mailing a paper Medicaid application, by phone at 1-855-889-7985, or in person, by email, or by fax at a local HHS office with an income maintenance worker. Then an assessor schedules a level of care assessment, Iowa Medicaid Medical Services decides level of care (denial comes by Notice of Decision with appeal rights), and an interdisciplinary team meeting builds the comprehensive service plan that authorizes services
