PACE HAWAII PROGRAM

PACE HAWAII PROGRAM provides multidisciplinary care to eligible adults aged 55 and older who need nursing facility-level care, including primary medical care, adult day health services, therapy services, home care, and non-emergency transportation through the Department of Human Services.

  • Does the person receiving care live in Hawaii? — Residency is usually the first thing a program checks.
