Pediatric Personal Care Benefit¶
Pediatric Personal Care Benefit provides in-home personal care services to Colorado Medicaid beneficiaries 20 years of age and younger with disabilities.
Eligibility¶
- Care recipient must have a qualifying disability
- Applicant age: under 20

- Personal care services must be pre-approved through a Prior Authorization Request (PAR); the personal care provider submits a completed Personal Care Assessment Tool (PCAT) and a 485 Home Health Certification and Plan of Care Form (completed by the client's doctor) in an online PAR
