Alaska Community First Choice (CFC)

Community First Choice (CFC) is a 1915(k) state plan option that provides long-term services and supports (LTSS) to eligible Alaskans. Services include Personal Care Services, Personal Emergency Response Systems (PERS), case management, chore services, and skill building. It emphasizes participant autonomy and allows the State to receive a higher federal match.

FY2026 Rate Schedule (effective July 1, 2025): - Agency-Based PCA (T1019): $9.01 per 15 min - Consumer-Directed PCA (T1019 U3): $9.01 per 15 min - CFC Personal Care (S5125): $9.01 per 15 min - CFC Chore Services (S5120 SE): $9.43 per 15 min - CFC Skill Building (S5108): $11.66 per 15 min Note: Regional multipliers apply (e.g., Arctic 1.48x, Bethel 1.49x, Aleutian 1.50x).
